Gynecomastia is a swelling of the breast tissue in boys or men, caused by an imbalance of hormones estrogen and testosterone. Gynecomastia can affect one or both breasts, sometimes unevenly. Neonates, adolescent boys, and older men may develop gynecomastia as a result of normal changes in hormone levels, although there are other causes as well.
In general, it is not a serious problem, but it can be difficult to cope with the disease. Men and boys with gyne sometimes have chest pain and may feel embarrassed.

What are the signs and symptoms of Gynecomastia?

How Should one Prepare for the surgery?

Gynecomastia
Gynecomastia is a medical condition, which refers to the development of the male breast tissue. It is an embarrassing condition which has seen an upward trend in the recent past to the tune of almost 30% of all men now.

It commonly occurs during puberty and can either be dormant or can increase with age and with weight gain.

Occasionally, gynecomastia may be associated with a dull aching kind of a pain.

Many men come to us when they feel embarrassed for many years and the common things that they tell me are that they are unable to swim, take off their shirt in front of others, need to wear loose shirts, have drooping shoulders and experience occasional pain.

Many a times, they are embarrassed to tell their own family members and they come for a consultation alone.

Causes of Gynecomastia
Gynecomastia is triggered by a decrease in the amount of the hormone testosterone compared to estrogen. The cause of this decrease can be conditions that block the effects of or reduce testosterone, or a condition that increases estrogen levels. Several things can upset the hormonal balance, including the following.

Natural hormone changes
The hormones testosterone and estrogen control the development and maintenance of sex characteristics in both men and women. Testosterone controls male traits, such as muscle mass and body hair. Estrogen controls female traits, including the growth of breasts.

Most people think of estrogen as an exclusively female hormone, but men also produce it — though normally in small quantities. Male estrogen levels that are too high or are out of balance with testosterone levels can cause gynecomastia.

Gynecomastia in infants : More than half of male infants are born with enlarged breasts due to the effects of their mother’s estrogen. Generally, the swollen breast tissue goes away within two to three weeks after birth.
Gynecomastia during puberty : Gynecomastia caused by hormone changes during puberty is relatively common. In some cases, the swollen breast tissue may go away without treatment within six months to two years. However, in majority of cases the gynecomastia persists and may actually increase in size.
Gynecomastia in men : The prevalence of gynecomastia in older individuals peaks again between the ages of 50 and 69. At least 1 in 4 men in this age group are affected.

Medications
A number of medications can cause gynecomastia. These include:

Malnutrition and starvation When your body is deprived of adequate nutrition, testosterone levels drop, but estrogen levels remain constant, causing a hormonal imbalance. Gynecomastia can also occur once normal nutrition resumes.

Herbal products
Plant oils, such as tea tree or lavender, used in shampoos, soaps or lotions have been associated with gynecomastia. This is probably due to their weak estrogenic activity

Different types of gynecomastia
There are several gynecomastia types; it may be one or two-sided, true or pseudogynecomastia, and the swelling can range in size or grades. Gynecomastia usually affects both sides, but one-sided or unilateral gynecomastia can also occur. It is common for one side to start growing before the other, and for one side to be slightly larger than the other.

True gynecomastia – where the breast tissue is enlarged – is also different from pseudogynecomastia. Pseudogynecomastia is the condition where a man appears to have breast tissue, or man boobs, but the swelling is caused entirely by fatty tissue. The breast itself remains the normal size. This is much more common in men who are overweight or obese.

Naturally, gynecomastia does not simply appear overnight, and different stages of gynecomastia can therefore be seen as the breast tissue grows.

For bilateral true gynecomastia, the American College of Plastic Surgeons has defined 4 different grades of gynecomastia.

These are important as they allow doctors to communicate with each other and document the growth or progression of the gynecomastia.

However, gynecomastia type or grade does not always match up with how concerned a man may be about his man boobs. Grade 1 is described as mild, but some men may find even this degree of gynecomastia extremely embarrassing or distressing.

Conversely, other men may find they can live quite happily with gynecomastia described as stage three or four.

Grade One – Mild enlargement of the breast bud, concentrated behind the areola
Grade Two – Breast growth spreading beyond the areola, with edges that blend with the chest wall
Grade Three – Breast growth spreading beyond the areola, with clear edges and redundant skin
Grade Four – Marked breast enlargement with redundant skin and feminization of the breast
Grade 1

When thinking about the different stages or types of gynecomastia, grade one is the first that men will notice. Some itching around the nipple area may precede it, or ‘jutting out’ of the nipple, but this is the first noticeable change most men see. Grade one can be felt as a firm or rubbery nodule behind the nipple. This does not spread beyond the edges of the pink ‘areola’ on the chest wall. It is usually found on both sides, and there may be some discomfort, but is not usually acutely painful. Many men may be happy to live with this degree of gynecomastia, although for some it may be very distressing.
If the cause of breast enlargement continues, the gynecomastia may progress through to other grades, although for some men grade one is as large as the swelling becomes.

Grade 2

Grade two describes where the breast tissue continues to grow and spreads beyond the pink areola. The swelling on the chest wall will become more noticeable. The edges of the gynecomastia remain indistinct, blending in with the rest of the chest wall. This grade is described as anywhere between mild and severe depending on the amount of growth.

Grade 3

Grade three gynecomastia is more obvious. Here the edges of the breast become clear, with a distinct ‘angle’ in the skin at the start of the breast enlargement. In addition, in grade three there will be some redundant skin. This means that if the man chooses to have surgery to get rid of his gynecomastia, he may need to have this skin removed to provide the best cosmetic outcome. This makes the operation a longer and more technical procedure, but may provide the best long-term results.

Grade 4

Grade four is the highest grade of gynecomastia. Here, in addition to the features described above, the breast is ‘markedly’ enlarged and there is feminization of the breast. This may mean that the breast starts to ‘sag’ or that there are changes to the nipple, making it appear more female. Surgery for this grade of gynecomastia will almost always involve removal of some redundant skin to achieve good aesthetic results. Treatments other than surgery are unlikely to provide a good result with this amount of breast enlargement.

Treatment
Surgery is the only way gynecomastia can be treated effectively. There are no drugs and exercises that can affect the reduction of a male breast.

In fact, many of the patients I get to see tell me that they have tried working out in the gym with an aim of getting rid of the gynecomastia, but feel that it has increased considerably – this is because the pectoralis muscle becomes bulky and it pushes the gynecomastia further forwards – causing it to appear bigger than before.

Pre-Operation Investigations
Prior to the operation, a few blood tests, x-ray chest and an ECG are performed so as to determine the fitness for anesthesia.

Surgery
Gynecomastia correction is a day care surgery, i.e. a few hours in the hospital is all that is required.
Depending on the grade, the procedure is performed with either local anesthesia or general (fully unconscious) anesthesia.
Essentially a Grade 1 can be easily corrected with use of local anesthesia, in which patients are fully awake and medicine is injected into the area which needs to be corrected – this area becomes numb, allowing us to perform the procedure.
Grade 2-4 will essentially require general anesthesia, in which the patient will be fully unconscious.
Surgery involves sucking out the fat with liposuction and removal of the fibrotic glandular structures with an excision procedure at the nipple.
Both require small holes (less than 4 mm) for the procedure to be performed.

Scar less Surgery
For certain individuals who are very particular that there should not be any scars at all, we devised a modification of the conventional surgery in which the entire procedure is performed with a single scar on the nipple (trans-nipple) and transsternal liposuction is performed.

The nipple heals without any scarring, so essentially there will be no other mark.

Recovery
Recovery is very quick – 2-3 days rest is all that is required for getting back to the normal routine.
